The ARMY MEDICAL DEPARTMENT insignia is hand-painted.

It says DR.BALL, so I think it's a coat that a doctor wore.
There are several spots where paint has been splattered on the laurel wreath peony in red (not blood), blue, orange, and other colors.
It has been cleaned and is in a fresh, good condition.
Size indicated as 44R
It's a generous size, but that means you can layer clothes underneath, so I think it's fine to choose a larger size.
